What to do if you lost your insurance policy document?

As a first step, you should approach the respective branch from where you got your document. Then you are required to get certification from the notary public for your address proof, and I.D proof along with at least any one of the premium payment receipt. You are required to pay Rs. 75/- as service charge in addition to the payment of rupees in the ratio of 20paise for Rs.  1000 for the total coverage amount... If you fulfill the above requirements, you will be given two forms.  One should be typed along with document for the value of Rs. 80/- and the other one, that is in the form of questionnaire should be filled properly and got attested from the notary public are to be submitted.

WITH IN FIFTEEN DAYS OF YOUR SUBMITION OF REQUISITION, YOU WILL BE GIVEN THE DOCUMENT.
